This independently contracted position will provide a 24/7 family-like setting for an individual. REQUIREMENTS: High School Diploma/GED Valid Driver's License Previous experience working as a Direct Support Professional is required Will need to successfully complete trainings through Community Options Must be open to supervisory visits in your home Our Family Model Providers are contracted and reimbursed at a per diem rate PLUS they receive monthly room and board from the individual they support – this income is tax exempt.
